Grant Amount: $165,000
AWARE Inc. will expand its existing Program for Assertive Community Treatment (PACT) team in central Montana. The counties it will serve include Lewis and Clark, Jefferson, Broadwater, and Meagher. PACT is the core service of a tiered delivery system that targets people with severe disabling mental illness who require long-term, intensive support that exceeds traditional outpatient services. The goal of the service is to rehabilitate each client to their highest level of functioning. PACT teams are mobile and deliver services in community locations to meet clients where they are. Grant funds will provide technical assistance from the National Council for Behavioral Health and support for part of the startup costs of expanding PACT services. AWARE has already established partnerships with many of the health and social service organizations that work in the region including, St. Peter’s Hospital, PureView, and the Helena Housing Authority, among others. Through this project, AWARE will deepen its current partnerships and develop new ones with the local health departments, clinics, housing authorities, shelters, and other entities as they expand into other counties. The project aims to stabilize 196 clients with severe disabling mental illness and reduce inpatient admissions.
