Grant Amount: $125,000

Great Falls Clinic will integrate behavioral health into their two primary care clinics and one specialty clinic, with Alluvion Health providing technical assistance and training. This project establishes a new partnership between Great Falls Clinic and Alluvion Health and leverages each organization’s strengths and resources. Alluvion’s experience with implementing the integrated behavioral health model positions them as a robust technical assistance partner. Funding will be used for Alluvion to hire a care coordinator and a behavioral health clinician and will assist with project oversight by organizational leadership. The project will also support consultation by the National Council for Behavioral Health. Ultimately, the project hopes to establish a sustainable integrated behavioral health model in the Great Falls Clinic and create a new successful partnership that will help improve patient health and reduce costs to the health system.
