Community Medical Center Integrated Behavioral Health Implementation
This project will integrate behavioral health services into Community Medical Center’s (CMC) primary care clinics and home-based primary care program. The grantee will accomplish this by hiring and embedding licensure-track social workers in to CMC’s Missoula, Frenchtown, and Lolo clinics. With consultation from the National Council for Behavioral Health, the primary care teams will provide universal screening, evidence-based treatment, and data tracking to improve outcomes and allow for care coordination across the hospital platform. Grant funding will be used to hire a behavioral health manager to oversee the project, a new behavioral health clinician, and additional clinical supervision for clinicians. Partners include the University of Montana who will help with the placement of behavioral health student interns and recent graduates.
