Strengthening the Circle: Increased Capacity for Increased Health
All Nations Health Center's Strengthening the Circle project was designed to increase the center's capacity to provide high-quality, integrated health care services to American Indians living in and around Missoula. The project implemented a holistic care model that improves coordination and collaboration within and between care teams. This included allocating staff time to design and execute the model, providing training to clinical and non-clinical support staff, and delivering high-quality technical assistance to support the successful implementation of the model. All staff were trained to use the electronic health record system, case conferencing, and co-location to provide patients with holistic rather than siloed care. Population health data and analysis are used to inform planning efforts. By providing integrated patient care that is reimbursable under Medicaid and Medicare, the health center is on track to increase annual revenue by at least 75%. All Nations' partnership with Greenway Health, a health information technology services company, will provide ongoing resources and support to maintain the progress that has been made and identify future opportunities for improvement.
